Liability Form

In order for a child to attend CHOSEN, a liability form must be filled out by a parent/guardian of each child.

Please share this link in your registration forms and ensure this form is filled out prior to arriving at camp.

Pastors/Main Leaders:

If you are only attending and not using accommodations, please ensure you bring ONE leader per every TEN children to ensure safety numbers.

Booking Accommodations:

Accommodations Safety Numbers:

For safety reasons, those using accommodations need to have 2 adult leaders (18+) for every 10 children or a minimum of 1 leader per 4 children of the same gender. Mixed gender rooming or supervision is not allowed.
